2. The entrant is deemed to be the holder of the established email address associated with the entry.

3. The entrant submitting the entry must be the 18 years of age or older.

By submitting the entry, entrant agrees and hereby releases Jay J Garcia, MD Weight Management & Wellness Centers and its parent companies, affiliates, subsidiaries, employees, agents, officers and directors from any and all liability or responsibility arising in connection with entrant's participation in the contest or as to any prize(s) which may be awarded.

Jay J Garcia, MD Weight Managment & Wellness Centers reserve the right at its sole discretion to reject and disqualify any entrant based on medical inclusion, medical conditions, or any other medical reason in which the physician deems would make participation in the program unsafe, unnecessary, or against physician discretion.

6. Jay J Garcia, MD Weight Management & Wellness Centers assumes no responsibility for entries unable to be processed for any technical reason and further reserves the right to cancel the contest if it becomes technically corrupted or because of unauthorized human intervention. Jay J Garcia, MD Weight Management & Wellness Centers reserves the right in the event of such cancellation to award the prize to an eligible, non-suspect entry received prior to cancellation.

7. This contest is limited to US Residents only, living in the Tampa Bay area who can attend weekly visits to one of the 5 local centers. Applicable fees are the responsibility of the entrant for any additional products or services the entrant may purchase during their new patient visit that is not specifically identified in this contest.  

8. Employees of Jay J Garcia, MD Weight Management & Wellness Centers and its affiliates, parent companies and subsidiary companies, and their immediate families (parents and children) are not eligible to participate in the contest.
